Hospice Liaison

JourneyCare Hospice Serenity Hospice is seeking a driven, high-impact Hospice Liaison to grow our presence in Crystal Lake, IL. We’re looking for a results-oriented sales professional with experience in hospice, home health, or healthcare marketing who knows how to build relationships and generate referrals.

About the role

In this field-based role, you will take ownership of your assigned territory (Crystal Lake, IL and surrounding areas), developing strong referral partnerships, increasing census, and positioning JourneyCare Hospice as the provider of choice. This position is ideal for someone who thrives on accountability, visibility, and winning new business.

Responsibilities

• Coordinates and facilitates referrals to hospice care based on hospice admission criteria.
• Assists in information gathering, education, and form completion aspects of patient intake, admissions, and transition to home.
• Develops consultative relationships in assigned accounts by making effective customer contacts and presentations.
• Develops and implements marketing and education programs to increase awareness of the company.

Requirements

• Bachelor’s Degree in marketing, business administration, or other related fields is preferred. An acceptable combination of experience and/or training may be considered in lieu of formal education.
• Minimum of one year of professional sales experience preferred.
• Experience in the hospice and healthcare industry is a plus.
• Demonstrates good verbal and written communication and organization skills.
• Valid driver’s license with an automobile that is insured in accordance with state requirements.
• Strong customer service skills and ability to maintain good relationships with customers, co-workers, and other business partners.
