About the Role

We are looking for a Hospice Liaison to work Full Time 40 hours weekly, in this position, you will be responsible for new referrals to the Agency in the Weymouth area. Serve as the primary liaison to prospective patients and designated health care providers as well as insurance case management professionals and community groups to ensure safe and appropriate care for all new patients.

Schedule

Tuesday - Saturday 8:00 am - 5:00 pm

How You'll Contribute

  • Build and maintain relationships with physicians, hospital staff, referral sources, patients, and families to facilitate the hospice referral and admission process.
  • Educate patients, families, and healthcare partners on Croí Health's hospice services and available care options.
  • Develop new referral relationships while collaborating with the Intake team to coordinate timely admissions.
  • Assess patients' clinical, social, and psychosocial needs to ensure they meet hospice admission criteria and coordinate appropriate transitions of care.
  • Collaborate with referral sources on complex discharges and obtain necessary clinical and financial information to support admissions.
  • Foster strong relationships with referral partners by providing exceptional service, responding to inquiries, and supporting growth within existing accounts.
  • Participate in community outreach, networking, and professional organizations to promote Croí Health's hospice services.
  • Track referral activity, admissions, and payer data, and maintain accurate reports to support agency goals and performance.

What Makes You a Great Fit

  • Minimum of 2 years of hospice experience.
  • Experience working with hospitals, physicians, case managers, discharge planners, assisted living communities, or other healthcare referral sources.
  • Strong knowledge of hospice services, Medicare hospice benefit, and the referral/admission process.
  • Excellent communication, relationship-building, organizational, and time management skills.
  • Current Massachusetts RN or LPN license preferred.
  • Community healthcare, Intake or sales experience preferred.

Pay

$75,000 - $90,000 (based on experience and credentials) plus bonus!
