About the role
Provides hospice care services to patients and families in their homes. Works closely with interdisciplinary teams to coordinate comprehensive care plans.
Responsibilities
- Administers medications and treatments as prescribed
- Assesses patient needs and provides ongoing care coordination
- Communicates with healthcare providers and family members to ensure continuity of care
- Trains and supervises hospice aides and volunteers
Requirements
- Bachelor's degree in a healthcare-related field
- Current CPR certification
- Experience in hospice or home health care preferred
Qualifications
- Excellent communication skills
- Strong organizational and time management abilities
- Ability to work independently and as part of a team
Skills
- Knowledge of hospice care protocols
- Proficiency in medical terminology
- Effective problem-solving and decision-making skills
