Hospice Community Liaison

About the role

Provides hospice care services to patients and families in their homes. Works closely with interdisciplinary teams to coordinate comprehensive care plans.

Responsibilities

  • Administers medications and treatments as prescribed
  • Assesses patient needs and provides ongoing care coordination
  • Communicates with healthcare providers and family members to ensure continuity of care
  • Trains and supervises hospice aides and volunteers

Requirements

  • Bachelor's degree in a healthcare-related field
  • Current CPR certification
  • Experience in hospice or home health care preferred

Qualifications

  • Excellent communication skills
  • Strong organizational and time management abilities
  • Ability to work independently and as part of a team

Skills

  • Knowledge of hospice care protocols
  • Proficiency in medical terminology
  • Effective problem-solving and decision-making skills
