Responsibilities
- Grows the census and represents the hospice program positively in the community.
- Attends and participates in relevant meetings and expos.
- Establishes and maintains ongoing contacts with social workers, discharge personnel, doctors’ offices, nursing homes, and other referral personnel.
- Proactively seeks opportunities to meet with referral sources and builds positive working relationships.
- Identifies and facilitates educational opportunities.
- Performs various activities with contracted skilled nursing facilities, personal care homes, assisted living facilities, senior housing communities, senior centers, and the like in targeted areas to increase awareness about the agency’s programs and services and build relationships.
- Develops and maintains ongoing, positive relationships with key contractors, discharge planners, and other facilities with a focus on providing attentive customer service.
Requirements
- 3+ years of leadership experience.
- 3+ years of marketing experience.
- Hospice preferred.
- Highly data-driven with the ability to manage and optimize marketing programs across channels (e.g., paid search, social, email).
- Demonstrated ability to work successfully on cross-functional teams.
- Excellent communication skills, both verbal and written, at all levels.
Qualifications
- Educates health care professionals, patients, and families about home care services.
- Develops and maintains relationships with key referral sources, such as hospitals, nursing homes, and physician offices to generate leads.
- Captures patient needs and identifies barriers to care through on-site visits.
- Coordinates patient admissions with the team.
- Maintains a complete, up-to-date record of targeted referral sources in territory in CRM.
- Responds to customer complaints in a timely manner.
