About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services, including volunteer and bereavement support. They ensure that services are integrated into the interdisciplinary plan of care.

Responsibilities

  • Supervise, evaluate, and coordinate the interdisciplinary team
  • Evaluate and approve patient admissions, assessments, and discharges
  • Serve as patient advocate and coordinator for community health care providers
  • Maintain and document patient records accurately
  • Ensure timely recertification decisions
  • Assure effective discharge planning
  • Manage team schedule and services 24/7
  • Facilitate interdisciplinary meetings and plan of care updates
  • Participate in on-call rotation
  • Interview, select, train, supervise, and dismiss team staff
  • Evaluate staff competence and performance
  • Oversee staff and volunteer schedules and schedules
  • Review and approve payroll
  • Monitor clinical records and team activities for accuracy and compliance
  • Implement performance improvement activities
  • Complete and implement patient/family satisfaction surveys
  • Support regulatory body surveys and requests
  • Perform chart reviews and utilization reviews
  • Address customer service issues and provide solutions
  • Visit LTC and Contract Bed facilities to ensure care plan integration and customer satisfaction
  • Provide inservices to customers and participate in professional development
  • Verify and approve patient care services and costs

Requirements

  • Reliable transportation with appropriate license and insurance
  • Private telephone in home
  • Working knowledge of oncologic nursing and palliative care
  • Understanding of interdisciplinary health care team dynamics
  • Knowledge of VITAS information system (Vx)
  • Two years supervisory experience or equivalent
  • Bachelor's degree preferred
  • Current R.N. License

Qualifications

  • Knowledge of primary medical and nursing care principles
  • Experience in supervising healthcare teams
  • Ability to work independently and manage multiple tasks
  • Strong communication and problem-solving skills
  • Ability to handle sensitive and confidential information
