Hospice Clinical Team Manager (RN Required)

About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services, including volunteer and bereavement support. They ensure services align with the interdisciplinary plan of care and maintain accurate patient records.

Responsibilities

• Supervise, evaluate, and coordinate interdisciplinary team members
• Evaluate and approve patient admissions and discharges
• Serve as patient advocate and coordinator for community health care providers
• Maintain and document patient records accurately
• Facilitate interdisciplinary meetings and update care plans
• Manage team schedule and provide on-call coverage
• Interview, select, train, supervise, and dismiss team staff
• Monitor and control team productivity and costs
• Assist with regulatory surveys and patient satisfaction surveys
• Address customer service issues and resolve grievances
• Participate in professional development and mentoring

Requirements

• Reliable transportation and appropriate license
• Basic knowledge of oncology and palliative care
• Understanding of healthcare team dynamics and end-of-life processes
• Working knowledge of VITAS information systems
• Two years supervisory experience in a healthcare setting
• Current RN license

Qualifications

• Reliable transportation with appropriate license and insurance
• Private telephone in home
• Knowledge of primary medical and nursing care principles
• Understanding of interpersonal dynamics within healthcare teams
• Basic knowledge of end-of-life processes and bereavement
• Working knowledge of VITAS information system
• Two years supervisory experience or equivalent
• Bachelor's degree preferred
