About the role
The Team Manager leads the Patient Care Team, overseeing all patient care services and ensuring seamless coordination of interdisciplinary care.
Responsibilities
- Supervise, evaluate, and coordinate the Patient Care Team
- Evaluate patient admissions and discharges for eligibility and appropriateness
- Serve as patient advocate and coordinator for community health care providers
- Maintain and document patient records accurately
- Assure continuity of care from admission to discharge or transfer
- Facilitate interdisciplinary meetings and update care plans
- Manage team schedule and provide on-call support
- Interview, select, train, supervise, and dismiss team staff
- Monitor and approve payroll, ensuring accurate time and expenses
- Ensure compliance with regulatory standards and conduct quality improvement activities
- Conduct chart reviews and participate in outcomes management
- Address customer service issues and provide inservices to customers
- Control costs and manage productivity within budget
- Attend professional development opportunities and achieve personal growth goals
Requirements
- Reliable transportation with appropriate license and insurance
- Private telephone in home
- Knowledge of primary medical and nursing care principles
- Understanding of interdisciplinary health care team dynamics
- Working knowledge of VITAS information system
- Two years supervisory experience in a health care setting
- Bachelor's degree preferred
- Current R.N. license in the state where the VITAS program is located
Qualifications
- Knowledgeable about the process of dying and bereavement
- Ability to analyze customer service issues and improve performance
- Experience with professional development and mentoring
