Hospice Care Planner

About the role

The Hospice Care Planner evaluates and develops care plans for patients referred for Hospice services, ensuring seamless admission from hospital discharge to appropriate programs that meet patient and family needs. They facilitate referrals, coordinate with medical staff, and serve as a key liaison within the healthcare team.

Responsibilities

• Build and maintain relationships with physicians, social workers, case managers, and discharge planners.
• Evaluate and develop care plans for patients referred for Hospice services.
• Ensure seamless admission from hospital discharge to appropriate programs.
• Facilitate referrals and coordinate with medical staff.
• Serve as a key liaison within the healthcare team.

Requirements

• Current NYS Registered Nurse license.
• Minimum of two years hospice nursing experience.
• Proficiency in physical assessment and interviewing.
• Excellent communication skills.
• Advanced critical thinking and problem-solving abilities.
• Ability to work independently.
• Bilingual Spanish/English abilities (depending on location).

Qualifications

• Bachelor's Degree preferred.
