About the role
The Hospice Care Planner evaluates and develops care plans for patients referred to Hospice services, facilitating admissions from hospital discharge to ensure appropriate care that meets patient and family needs. They work on-site at a major medical center, building and maintaining relationships with physicians, social workers, case managers, and discharge planners to support seamless patient transitions.

- Support seamless patient transitions
Requirements
- Current license as a Registered Professional Nurse in NYS
- Bachelor’s Degree preferred
- Minimum of two years of hospice nursing experience
- Strong physical assessment and interviewing skills
- Excellent communication and client-service focus
- Advanced critical thinking and problem-solving abilities
Skills
- Bilingual Spanish/English skills (required in some locations)
