Hospice Care Planner

About the role

Evaluate and develop personalized care plans for patients referred to Hospice services, facilitate admissions from hospital discharges, and ensure care aligns with patient and family needs.

Responsibilities

• Build and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center to support seamless patient transitions.

Requirements

• Current NYS Registered Professional Nurse license.
• Bachelor’s Degree preferred.
• At least two years of hospice nursing experience.
• Strong physical assessment and interviewing skills.
• Excellent communication abilities.
• Bilingual Spanish/English skills required in some locations.
