Hospice Care Planner

The Hospice Care Planner is responsible for evaluating and developing care plans for patients referred to Hospice services, ensuring seamless admission from hospital discharge. They facilitate referrals, matching patient needs with appropriate Hospice programs, and work closely with medical staff to coordinate care. The role involves building and maintaining relationships with physicians, social workers, case managers, and discharge planners to support patient and family needs. Requirements include a current NYS Registered Nurse license, a minimum of two years of hospice nursing experience with strong assessment and interview skills, and excellent communication abilities. Advanced critical thinking and independent work skills are essential, with bilingual Spanish/English proficiency preferred in some locations. This position focuses on providing compassionate, client-centered end-of-life care within a hospital setting.
