About the role
ADVOCAP's Homeless Prevention program is seeking a Homeless Prevention Specialist for our Berlin office. The program serves Fond du Lac, Green Lake, and Winnebago counties. We assist individuals and families to overcome homelessness and/or potential homelessness and poverty by establishing and implementing an individual plan for them to achieve self-sufficiency, especially housing stability.
Responsibilities
- Respond to individuals and families in crisis and assist them in addressing basic needs
- Work with individuals and families in establishing their plan for self-sufficiency
- Coach participants along their path to self-sufficiency
- Connect participants to appropriate community resources and document the individual's needs, plans, services provided, and outcomes achieved
- Identify gaps in community services
- Maintain records documenting participant progress
- Assess and enroll participants into appropriate programs
- Establish trusting relationships with people
Requirements
- Bachelor's degree in human services or social services, and 2 years of previous case management work experience or an equivalent combination
- Understanding of poverty and ability to be non-judgmental with those served
- Ability to develop positive, growth-directed relationships with individuals and families in poverty
- Knowledge of and experience in utilizing community resources to help people in need
- Acceptable drug screen and comprehensive background check required
- Reliable attendance and a professional work ethic
