HomeCare Care Team Coordinator

Job Summary

The Care Team Coordinator (CTC) is responsible for the daily operations of the patient care team within HomeCare & Hospice. The CTC coordinates the day, evening and weekend operations of the care team, works in conjunction with the Division Manager to develop and monitor patient care services based on customer needs, and is responsible for clinical outcomes and patient satisfaction for designated team(s).

Responsibilities

• Coordinate and schedule all clinical care across the disciplines and health care providers.
• Develop and monitor patient care services based on customer needs.
• Facilitate the provision of high-quality patient care and services.
• Participate in performance improvement initiatives of the organization.
• Oversee new employee orientation, on-going staff competencies, and assist with 90 day and annual performance reviews.
• Accountable for employee engagement for designated team(s).

Requirements

• Five years clinical experience required.
• Three years’ experience in the home care or hospice setting preferred.
• Registered Nurse, BSN required.
• MSN or equivalent preferred.
• Licensed in Commonwealth of PA OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact.
• CPR Certification.
• Current Driver’s license and proof of auto insurance required.
• Specialty nursing certification preferred.

Qualifications

Education: Registered Nurse, BSN required. MSN or equivalent preferred.
