As a Licensed Practical Nurse (LPN), you’ll play a critical role in providing compassionate, patient-centered care in a home health setting. Working closely with Registered Nurses and other healthcare professionals, you'll support patients in their journey toward recovery and quality of life.

Responsibilities

• Provide Exceptional Care: Implement care plans under the direction of Registered Nurses, ensuring the highest standards of patient care.
• Empower Patients: Assist clients with preventative and rehabilitative nursing procedures to improve their health and well-being.
• Collaborate with a Caring Team: Build strong relationships with patients, families, and healthcare providers to deliver coordinated care.
• Monitor Care Quality: Ensure that services are delivered with attention to quality and appropriateness.
• Follow Safety Protocols: Maintain a safe, clean environment by adhering to infection control and organizational policies.

Requirements

• Education & Licensing: Graduation from an NLN-accredited LPN program with active New Jersey LPN licensure.
• Experience: 2+ years of med/surg or acute care nursing experience. Home care or hospice experience is a plus.
• Skills: Strong verbal and written communication skills, excellent computer skills, and exceptional organizational abilities.
• Team Player: Ability to collaborate and foster positive, professional relationships.
• Driver’s License & Transportation: Valid driver’s license and reliable transportation for local travel.

Pay

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $26.00 to USD $34.00 per hour.
