As a Licensed Practical Nurse (LPN), you’ll play a critical role in providing compassionate, patient-centered care in a home health setting. Working closely with Registered Nurses and other healthcare professionals, you'll support patients in their journey toward recovery and quality of life.

Responsibilities
- Provide Exceptional Care: Implement care plans under the direction of Registered Nurses, ensuring the highest standards of patient care.
- Empower Patients: Assist clients with preventative and rehabilitative nursing procedures to improve their health and well-being.
- Collaborate with a Caring Team: Build strong relationships with patients, families, and healthcare providers to deliver coordinated care.
- Monitor Care Quality: Ensure that services are delivered with attention to quality and appropriateness.
- Follow Safety Protocols: Maintain a safe, clean environment by adhering to infection control and organizational policies.
Requirements
- Education & Licensing: Graduation from an NLN-accredited LPN program with active New Jersey LPN licensure.
- Experience: 2+ years of med/surg or acute care nursing experience. Home care or hospice experience is a plus.
- Skills: Strong verbal and written communication skills, excellent computer skills, and exceptional organizational abilities.
- Team Player: Ability to collaborate and foster positive, professional relationships.
- Driver’s License & Transportation: Valid driver’s license and reliable transportation for local travel.
Pay
In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $26.00 to USD $34.00 per hour. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
