Responsibilities

• Provide exceptional care by implementing care plans under the direction of Registered Nurses.
• Empower patients through preventative and rehabilitative nursing procedures to improve health and well-being.
• Collaborate with patients, families, and healthcare providers to deliver coordinated care.
• Monitor care quality to ensure services meet high standards of appropriateness.
• Follow safety protocols, including infection control and organizational policies, to maintain a safe environment.

Requirements

• Graduation from an NLN-accredited LPN program with an active New Jersey LPN license.
• 2+ years of med/surg or acute care nursing experience; home care or hospice experience is a plus.
• Strong verbal and written communication skills, excellent computer skills, and exceptional organizational abilities.
• Ability to collaborate and foster positive, professional relationships.
• Valid driver’s license and reliable transportation for local travel.

Pay

Compensation range for this position is USD $26.00 to USD $34.00 per hour. Offers are determined based on the scope and responsibilities of the role, candidate’s work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
