About the Role

The Home Therapy RN acts as a coordinator of patient care, collaborating with care teams to deliver essential services. They serve as a patient educator and advocate, promoting informed decision-making and patient independence.

Responsibilities

• Perform home visits to assess safety and suitability for home dialysis in compliance with CMS regulations.
• Identify opportunities for professional development, quality improvement, and research application.
• Address ethical challenges with minimal guidance.
• Support patients in managing their treatment and fostering independence.

Requirements

• Minimum one year of RN experience, including nephrology or dialysis experience.
• Graduation from an accredited nursing program.
• Active RN licensure and CPR certification.
• Proficiency with Electronic Health Records, organization, and independent project management.
• Valid driver’s license and reliable transportation for home visits.
