Key Responsibilities
- Conduct strengths-based assessments and identify/prioritize the needs of children and families across all domains.
- Provide field-based mental health services, case management, and crisis intervention (including 24-hour availability as needed).
- Develop and implement individualized Family Support Plans in collaboration with families, multidisciplinary teams, and community supports.
- Facilitate team meetings and ensure coordination among agencies, including DCFS, DMH, courts, schools, and healthcare providers.
- Complete all required documentation, reports, and evaluations in compliance with program guidelines and agency policies.
- Participate in supervision, trainings, and meetings while maintaining professional standards, flexibility in scheduling, and overall program support.
Qualifications
- Registered with the BBS as an AMFT, APCC, ACSW, LMFT, LCSW, and LPCC in the State of California
- Minimum one year of experience working with children, adolescents, and families
- Strong understanding of family-centered, strengths-based, and field-based service models, including group facilitation
- Ability to collaborate effectively as part of a multidisciplinary team
- Knowledge of local community resources and experience working with diverse populations preferred
- Valid California driver’s license with a clean driving record required
