About the Role

The Home Therapy RN coordinates patient care as a nurse and educator, collaborating with healthcare teams to provide home dialysis services. They conduct home visits to assess the environment's safety and suitability in compliance with regulations, promote patient independence, and facilitate informed treatment decisions.

Responsibilities

• Initiate patient care practices and act as a care coordinator
• Educate and advocate for patients, ensuring informed consent
• Perform home assessments for safety and appropriateness
• Engage in professional development, quality improvement, and ethical practice
• Collaborate with multidisciplinary teams to deliver personalized care
• Foster relationships to improve patient outcomes and quality of life

Requirements

• Minimum of 1 year RN experience and dialysis-specific nephrology nursing
• Graduation from an accredited nursing program
• Current RN license and CPR certification
• Proficiency with Electronic Health Records
• Valid driver’s license and reliable transportation for home visits
