Home Provider / Shared Living Provider

Role Overview As a Home Provider, you will open your home to support individuals with intellectual disabilities, fostering a sense of belonging, independence, and growth. Your responsibilities include assisting with daily living skills, medication management, community participation, transportation, and personal care support as needed. You will collaborate with a support team, maintain documentation, and help individuals build confidence and pursue personal goals. Qualifications & Skills Must be at least 21 years old and a resident of Illinois.Ownership or rental of a home, along with active insurance.Reliable transportation, valid driver’s license, and insurance.Internet access and basic computer skills.Ability to complete background checks, screenings, and training.Registered healthcare worker (DSP) preferred. Key Duties Create a safe, welcoming home environment.Support daily activities, including personal care, medication, and medical appointments.Encourage community involvement and independence.Maintain communication with support teams and document progress.Provide behavioral, medical, or specialized care as needed.
