Home Physical Therapist

About the Role

We are seeking a compassionate Orthopedic At-Home Physical Therapist to evaluate and treat patients recovering from orthopedic surgeries and conditions in their homes.

Responsibilities

• Educate patients on safe mobility and techniques for transfers and ambulation
• Conduct evaluations and develop individualized treatment plans
• Provide functional training for activities of daily living
• Document care and communicate with the interdisciplinary team
• Ensure safe, effective interventions aligned with best practices
• Demonstrate strong clinical decision-making and proficiency with EMR systems

Requirements

• Bachelor’s or Master’s degree in Physical Therapy (Doctorate preferred)
• Valid state licensure as a Physical Therapist
• Completed continuing education units (CEUs)
• Current CPR certification
