Homecare Physical Therapist

Responsibilities

• Provide compassionate, evidence-based rehabilitation to adult and geriatric patients in their homes.
• Conduct comprehensive evaluations and develop individualized care plans.
• Deliver therapeutic interventions to enhance mobility, safety, and independence.
• Collaborate with an integrated care team to ensure coordinated patient care.
• Document patient progress and care details in the EMR system.
• Contribute to quality improvement and process-efficiency initiatives.
