Provide evidence-based therapeutic interventions to enhance mobility, safety, and independence for adult and geriatric patients in their homes.

Responsibilities

• Perform comprehensive evaluations and develop individualized care plans for adult and geriatric patients in their homes.
• Provide evidence-based therapeutic interventions to enhance mobility, safety, and independence.
• Collaborate with an integrated care team to ensure coordinated patient care.
• Document treatments and progress accurately in the Electronic Medical Record (EMR).
• Participate in quality improvement and process-optimization initiatives.

Qualifications & Skills

• Strong clinical assessment and treatment planning skills.
• Ability to work independently in a home setting with adult and geriatric populations.
• Excellent communication and teamwork abilities.
• Knowledge of EMR documentation processes.
