Job Summary

The Home Navigator will meet with parents and families in the home environment to provide parent education, child development information, and family support. Utilizing the nationally recognized “Parents as Teachers” (PAT) curriculum, the Home Navigator promotes the health and development of children from pregnancy to kindergarten.

About the Role

Home Navigators utilize a trauma-informed approach to support families and recognize the impact of traumatic stress on children and their families. They establish and develop collaborative partnerships and connect families to resources.

Essential Functions

  • Provide in-home case management and utilize community resources to link families to appropriate services, conduct in-home teaching and supportive problem-solving.
  • Utilize the Parents as Teachers curriculum and engage parents in parent-child interaction activities, and increase knowledge of children’s development.
  • Work with adults and their young children to develop, monitor, and review goals with each family.
  • Establish and develop collaborative partnerships and connect families to resources.
  • Help parents and children transition to other services as needed.
  • Maintain and manage a caseload of approximately (15-20) families.
  • Complete an initial and annual family-centered assessment with each family.
  • Complete at least initial and annual developmental screening and health review for each enrolled child.
  • Establish a relationship with families and empower them by building on their strengths.
  • Maintain and submit all required family documentation and data in a timely manner.
  • Work independently and be self-motivated.
  • Participate in at least two hours of reflective supervision monthly and at least two hours of staff meeting monthly.
  • Obtain competency-based professional training and renew home navigator certification annually.
  • Facilitate group connections to give families an opportunity to build social connections with one another.

Requirements

  • Education: A bachelor’s degree in social work, Child Development, Psychology, or another related field is preferred; candidates with a high school diploma or equivalent and 2+ years’ work experience in home visitation may be considered in lieu of a degree.
  • Skills: Strong communication skills, written and verbal, interpersonal skills, planning/organizing, information gathering, identifying solutions, summarization, listening, goal setting, multi-tasking, teamwork, collaboration, initiative, and ability to make independent decisions within guidelines, and follow the code of ethics.
  • Experience: 2+ years’ experience working with high-risk families and families with young children, case management and a strong understanding of community resources required, Ability to work independently and be self-motivated, Must have experience working with a diverse population and be a team player.
  • Physical Demands: Working with children and their families may require the ability to sit on floors, stand, stoop, crawl, lift, carry, push, and pull.
