Essential Duties and Responsibilities
- Record keeping
- Maintain staff records relating to time, attendance, level of performance, specific deficiencies, counseling and discipline sessions, and training.
- Ensure that Staff Accident Forms are completed accurately and submitted on time.
- Maintain the Staff Data Base for all required trainings.
- Maintain residence and equipment
- In conjunction with the Agency Maintenance Department and netfacilities.com, assure that the home is well maintained and in good repair.
- Avoidance of maintenance issues by coordinating with the Agency Maintenance Department and netfacilities.com.
- Coordinate transportation/vehicle use to maximize efficient use of vehicles and staff.
- Complete at least two Environmental Assessments monthly as required, four per month if score below 90%.
- Make adjustments to maintain continuous improvement on scores.
- Maintain the emergency food and water supply.
- Maintain adequate stock of food, household and cleaning supplies.
Performance Factors
- Accountability
- Periodically meet with and counsel staff about performance, program, personnel, and/or training issues.
- Complete employee performance appraisals.
- Complete monthly Program System Reviews for all DSPs, Shift Coordinators and/or Assistant Home Managers assigned to the home.
- Review results and, if needed, assist staff to improve performance.
- Respond to program and work related questions, problems, complaints and concerns from staff.
- Make sure that DSPs know and correctly implement house rules and procedures.
Safety and Customer Service
- Working with staff, ensure that each individual we work for maintains a clean and neat appearance that is appropriate for home and for community outings.
- Avoidance of accidents by ensuring accurate records of possessions and clothing are maintained for people we work for.
- Conduct and review fire drills as per division requirements.
- Ensure correct implementation of mealtime plans including making sure that all needed items for meals are available and preparing meals according to mealtime plans.
- Notify the off-site program when a resident will be late or will not attend.
- Ensure that the DSP, and Shift Coordinator read the T-Logs and the S-Comm notes in Therap at the start of each shift.
- Know the likes and dislikes of each person in order to better plan quality activities and community events.
- Ensure that all people we work for arrive on time for medical appointments.
- Ensure that staff providing direct support in the home collect and record data accurately.
- Provide direct support services to people we work for as needed.
- Ensure that all staff completes General Event Reports and any necessary follow up review in Therap in a timely and accurate manner.
Supervisory Responsibilities
- Supervise at least 5 and as many as 20 Direct Support Providers.
- Assign tasks to DSPs to deliver service to specific individuals as outlined in the Individual Residential Habilitation Plan (IRHP) and the Individual Plan of Protective Oversight (PPO).
- Maintain a list of assigned responsibilities for overnight staff.
- Ensure that each DSP is assigned either primary or secondary responsibility for delivering service to specific individuals we work for.
- Ensure that all shifts have adequate coverage.
- Follow Employee Work Schedule.
- Obtain prior approval of Assistant Residential Director-Home Management before adjusting the Employee Work Schedule.
- Ensure that staffing in the home does not exceed the Full-Time Equivalency (FTE).
- With prior supervisory approvals, acceptable instances of exceeding FTEs are staff training, emergency care, etc.
Qualifications
- Associate's degree (A.A.) or equivalent from a two-year college or technical school; or one year related experience and/or training; or equivalent combination of education and experience.
- Language Skills Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups.
- Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio, and percentage and to draw and interpret bar graphs.
- Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variable in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
- Computer Skills Must be knowledgeable about Internet software, Microsoft Office components including Word, Office, Excel and PowerPoint. Becomes knowledgeable and trained in the use of Therap for plan writing, event reporting and ongoing communication.
- Certifications, Licenses, Registrations Must possess a valid NYS driver's license.
