Home Health - Weekday Split Shifts

We are seeking a Registered Nurse (RN) to provide care for a medically fragile pediatric patient in a home setting. Key responsibilities include utilizing the nursing process to plan, implement, and evaluate patient care, initiating and revising care plans based on patient needs, administering medications and treatments, and responding to changes in patient condition. The role involves caring for patients with complex needs such as g-tube feeding, seizure precautions, vents, and trachs, with opportunities for skills advancement through training. Qualifications include a valid RN license, current health certification, PPD or chest X-ray, BLS certification, and preferred experience of at least one year. Skills necessary are strong patient assessment, medication administration, care coordination, and effective communication. Key duties involve planning and delivering patient care, monitoring patient status, providing health education, and collaborating with healthcare teams.
