About the role
Provide skilled nursing care for medically fragile pediatric patients in a home setting, emphasizing compassionate, patient-centered care.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver prescribed nursing services and monitor patient condition.
- Identify physiologic and psychosocial changes, document diagnoses, and coordinate with the healthcare team.
- Care for patients with complex needs including g-tube, medication management, seizure precautions, and vent/trach dependence.
- Participate in skills advancement programs with hands-on training.
Qualifications
- Current LPN/LVN license in the practicing state.
- Valid BLS card, TB or Chest X-Ray.
Skills
- Strong assessment skills.
- Ability to follow prescribed orders.
- Effective communication with healthcare team.
