LPN - Weekday Split Shift

Role: Licensed Practical Nurse (LPN) providing skilled nursing care for medically fragile pediatric patients in a home setting. Responsibilities Include Assessing, planning, implementing, and evaluating patient care using the nursing process.Delivering prescribed nursing services and monitoring patient condition.Identifying physiologic and psychosocial changes, documenting diagnoses, and coordinating with healthcare team. Qualifications Current LPN/LVN license in the practicing state.Valid BLS card, TB or Chest X-Ray.Preferred: at least one year of professional nursing experience. Key duties include: caring for patients with complex needs such as g-tube, medication management, seizure precautions, vent/trach dependence, and participating in skills advancement programs with hands-on training. Skills required: strong assessment skills, ability to follow prescribed orders, and effective communication with healthcare team.
