Role Overview A Home Health Speech Language Pathologist provides personalized speech, voice, language, hearing, and swallowing therapy to patients in their homes or facilities. Reporting to the Clinical Manager, responsibilities include evaluating patients, developing and reviewing treatment plans, administering diagnostic tests, and monitoring patient progress. The role involves instructing patients and families on communication strategies, maintaining detailed documentation, and participating in care coordination and discharge planning. Maintaining confidentiality and adhering to professional standards are essential. Qualifications & Skills Minimum six months of experience as a speech therapist or speech-language pathologistCurrent and unrestricted licenseCPR certificationValid driver’s license, auto insurance, and reliable transportation Key Duties Provide speech/language pathology services and assessmentsDevelop, review, and implement treatment plansEvaluate and treat disorders, administer diagnostic testsTrain patients and provide counseling to patients, families, and healthcare staffDocument and report patient progress and coordinate care activitiesMaintain confidentiality and uphold professional standards Additional Information The role involves patient-facing activities, TB screening, and adherence to driver safety policies.
