Responsibilities

• Schedule and coordinate home health care services for patients
• Communicate with patients, families, and healthcare providers to ensure continuity of care
• Ensure compliance with regulatory requirements and patient safety standards
• Manage patient appointments and monitor progress towards treatment goals
• Provide administrative support to the home health team

Requirements

• High school diploma or General Education Degree (GED)
• One year experience scheduling and working with older adults or in a related healthcare setting
• Reliable transportation and proof of valid automobile liability insurance
• Valid driver’s license
