About the Role

As a Home Health Scheduler at Elara Caring, you will support Administrators (or Designated Branch Directors) in scheduling patient visits. You will advance key workflows throughout the patient journey, manage direct care team member schedules, and ensure adequate and timely patient care visits. Acting as an intermediary, you will collaborate with the Clinical Team Manager (CTM), direct care team members, the patient’s physician or medical provider, and other facility staff overseeing patient care.

Responsibilities

  • Schedule patient visits and manage direct care team member schedules to ensure timely and adequate care.
  • Act as an intermediary between the Clinical Team Manager, direct care team members, physicians, and facility staff.
  • Convey information and opinions clearly, listen actively, and respond with appropriate tone and messaging.
  • Set timeframes, match resources to tasks, and plan ahead to optimize workflows.
  • Make decisions with incomplete information, evaluate multiple options, and escalate when necessary.
  • Become proficient in enterprise applications (e.g., Homecare Homebase, Workday) and Microsoft Office.
  • Handle complaints, manage conflict, and negotiate outcomes effectively.
  • Seek patient feedback, advocate for patient needs, and manage patient expectations.
  • Respond to and control unexpected situations by evaluating solutions and implementing the best course of action.
  • Understand the financial impact of scheduling decisions and staff productivity metrics.

Requirements

  • High school diploma or GED required; Associate’s degree preferred.
  • 1 year of practical experience required.
  • 1 year of experience in patient scheduling within a home care agency setting is preferred.
  • Familiarity with national standards of care, state licensure, and federal conditions of participation.
  • Ability to quickly learn and navigate enterprise applications and intermediate knowledge of Microsoft Office.
  • Bilingual in Spanish and English is preferred.
