Position Overview

Seeking a Physical Therapist (PT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaborate with interdisciplinary teams, provide patient evaluations, develop treatment plans, and document progress effectively. Responsible for discharge planning, patient education, and supervising personnel such as PTA and HHA. Ensure compliance with policies, regulations, and documentation standards, including OASIS assessments where permitted. Maintain clear communication with staff, patients, and families, serving as a clinical resource.

Qualifications & Skills

• Degree in Physical Therapy from an accredited program
• Current, unrestricted state license
• Proficiency in assessments, documentation, and compliance
