Position Overview
Key Responsibilities
- Perform assessments of mobility, strength, and function
- Develop and document treatment plans with measurable goals
- Administer therapy services following physician orders
- Maintain accurate documentation to ensure compliance and optimize patient outcomes
- Cookordinate with healthcare team, educate patients and caregivers, and participate in discharge planning
- Supervise relevant staff and complete assessments like OASIS where permitted
Qualifications & Skills
- Degree in Physical Therapy from an accredited program
- Current, unrestricted state license
- Valid CPR certification
- Valid driver’s license and auto insurance
Preferred Experience
- Home health therapy experience
- OASIS documentation familiarity
