Position Overview
Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover at home.
Responsibilities
- Conducting initial and ongoing assessments
- Developing and implementing personalized plans of care
- Delivering prescribed therapy services
- Documenting patient progress
- Moderating treatment plans
- Collaborating with healthcare providers and families
- Supervising PTA and HHA staff (if applicable)
Requirements
- Degree in Physical Therapy
- Unrestricted state license
- CPR certification
- Valid driver’s license with insurance
Qualifications
- Prior home health experience preferred
- OASIS documentation skills preferred
