Position Overview

Key Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and function.
• Develop and document plans of care with goals, progress, and outcomes.
• Provide therapy services per physician orders and modify treatments as needed.
• Manage discharge planning and educate patients and caregivers on home exercise programs.
• Cook up coordination with healthcare team members and document accurately to ensure compliance.
• Supervise personnel (PTA, HHA) and complete assessments where permitted.

Qualifications And Skills

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license (WI and MN).
• Valid driver’s license, CPR certification, and proof of insurance.
