Home Health RN | Flexible Hours

About the role

Provides skilled nursing care in patients' homes, including assessments, care planning, medication management, patient education, and documentation using the Epic system.

Responsibilities

• Manage multiple patients with varying acuity levels
• Perform post-hospitalization assessments and discharges
• Conduct dressing changes and IV therapy
• Assist with safety and community resources
• Collaborate with healthcare teams to ensure continuity of care and adapt plans as needed
• Deliver patient-centered care and develop and implement care plans
• Monitor patient progress and educate patients and families
• Ensure safety in the home environment

Requirements

• Valid RN license in Missouri
• BLS certification
• Valid driver’s license and auto insurance

Qualifications & Skills

• Preferred: 1+ year in home health or acute care
• Strong communication, assessment, and documentation skills
