Home Health RN | Flexible Hours

Role Overview: Provides comprehensive skilled nursing care in patients' homes, including case management, post-hospital assessments, discharges, patient education, dressing changes, IV therapy, and general nursing tasks according to policies. Utilizes the Epic system for documentation and supports patient safety through additional home needs like lifeline and meal programs. Responsibilities: Deliver skilled nursing as prescribed, assess patient needs, develop and adjust care plans, monitor progress, educate patients and families, and collaborate with healthcare teams to ensure continuity of care.
