About the role
Skilled Nursing Registered Nurse (RN) providing comprehensive, patient-centered home health care.
Responsibilities
- Deliver skilled nursing care adhering to professional, regulatory, and organizational standards.
- Perform comprehensive patient assessments and develop individualized care plans in collaboration with physicians, families, and the interdisciplinary team.
- Execute physician orders, coordinate with therapy services, community agencies, and other healthcare providers.
- Utilize clinical judgment and critical thinking to make independent nursing decisions.
- Educate patients, families, and caregivers on disease management, medications, treatments, and health promotion.
- Maintain accurate documentation and communicate patient status changes effectively.
- Participate in on-call rotations, Quality Assessment, Performance Improvement activities, and staff training.
Qualifications & Skills
- Current Ohio RN licensure and BLS certification from AHA.
- Minimum one year of in-hospital or home care nursing experience.
- Proficiency with Microsoft Office; EMR experience preferred.
- Strong organizational, communication, critical thinking, and time management skills.
- Bachelor of Science in Nursing (BSN) preferred or to be obtained within five years.
Pay & Schedule
- Part-time role with a 0.7 FTE
- $5,000 sign-on bonus
- Working hours between 0800-1530 or 0900-1630
- Onsite presence required
