Home Health RN

Duties

• Complete OASIS assessments including Start of Care, Resumption of Care, Recertification, and Discharge visits.
• Deliver skilled nursing care and ongoing patient assessments in the home setting.
• Maintain accurate and timely clinical documentation.
• Cookordinate care with physicians, caregivers, and the interdisciplinary team.
• Participate in on-call rotation and ensure high-quality patient care.

  Requirements

  • Education: Associate Degree in Nursing (ADN) required.
  • Licenses: Active Kentucky RN License.
  • Certifications: BLS (AHA).
  • Experience: Minimum 1 year of RN experience.
  • Skills/Specialties/Technology: Home Health, OASIS, Case Management, EMR Documentation, Patient Assessment, Care Coordination, Critical Thinking.
