Responsibilities

• Assess patient needs and develop/update individualized care plans
• Administer medications and treatments
• Perform wound care and manage chronic diseases
• Coordinate with physicians, therapists, and family members to ensure safe transitions and optimal patient outcomes
• Provide skilled nursing care in patients' homes and support safe care transitions
• Contribute to improved patient well-being in a supportive environment

• Compassionate bedside manner and strong clinical skills
• Ability to assess, plan, and execute patient care
• Effective communication and collaboration skills
• Knowledge of wound care and chronic disease management
