Dependable Home Health of Tucson is seeking a compassionate and dedicated Registered Nurse (RN) to provide home health care to patients primarily within the 85706 and 85756 zip codes. This is a great opportunity for an RN who wants a more focused geographic territory, meaningful one-on-one patient care, and the independence of working in home health while still having the support of an experienced clinical team.

Responsibilities

• Conduct comprehensive in-home patient assessments
• Develop and update individualized plans of care
• Administer medications, treatments, injections, and wound care according to physician orders
• Monitor patient progress and recognize changes in condition
• Communicate patient updates and concerns with physicians and the interdisciplinary care team
• Educate patients and caregivers on medications, disease management, safety, and wellness
• Coordinate care with therapists, social workers, physicians, and other healthcare professionals
• Supervise and document care provided by LPNs and Home Health Aides (HHAs), as appropriate
• Maintain timely, accurate, and compliant clinical documentation
• Help patients maintain their safety, independence, and quality of life at home

Requirements

• Current Arizona RN license in good standing
• Minimum of one year of RN experience preferred
• Home health, hospice, rehabilitation, med-surg, or critical care experience is a plus
• Current CPR certification
• Valid driver’s license and reliable transportation
• Strong clinical assessment and critical-thinking skills
• Excellent communication, organization, and time-management skills
• Ability to work independently while collaborating with an interdisciplinary team
