Home Health Registered Nurse -South West Tucson & GV/Sahuarita

About the role

Perform in-home patient assessments and create personalized care plans.
Administer medications, wound care, and treatments per physician orders.
Educate patients and caregivers on condition management and independence.
Monitor and document patient progress, reporting updates to the care team.
Supervise LPNs and Home Health Aides to ensure quality and compliance.
Participate in interdisciplinary meetings to coordinate holistic care.

Requirements

• Active Registered Nurse (RN) license in Arizona.
• Minimum of one year of nursing experience (home health, hospice, or critical care preferred).
• CPR certification and valid driver’s license with reliable transportation.
• Excellent clinical judgment, communication, and time-management skills.
