Home Health PT - Full Time

Seeking a Physical Therapist (PT) for home health services. You will assess, teach, and deliver therapy interventions based on physician-directed plans of care, collaborating with interdisciplinary teams to optimize patient outcomes. Key Responsibilities Include Conducting initial and ongoing assessments of mobility, strength, and functionAdministering tests, interpreting results, and setting functional goalsProviding written evaluations and treatment plans, documenting progress, and adjusting care as neededManaging discharge planning and educating patients and caregivers on home exercise programsCoordinating with healthcare team members and supervising applicable personnelCompleting required assessments and ensuring documentation compliance Qualifications Degree in Physical Therapy from an accredited programCurrent, unrestricted state licenseProficiency in clinical assessments, documentation, and policy adherenceValid CPR and driver’s license Preferred: Home health experience and OASIS documentation skills.
