Home Health PT - Full Time

Position Overview A full-time Physical Therapist (PT) is needed to join a home health care team, facilitating patient recovery in their own homes. Key Responsibilities Conduct initial and ongoing assessments to evaluate mobility, strength, and functional abilities. Administer and interpret tests to set functional goals and outcomes. Develop and document personalized plans of care, including treatment modifications and discharge planning. Deliver physical therapy services following physician orders, ensuring proper documentation and compliance. Collaborate with healthcare team members, educate patients and caregivers, and supervise personnel such as PTAs and HHAs as applicable. Complete OASIS assessments where permitted, adhere to policies, standards, and regulations. Maintain effective communication within the team and with patients’ families. Qualifications Degree in Physical Therapy from an accredited program. Current, unrestricted state license. Valid CPR certification. Preferred Skills Strong organization, communication, and time management skills. Attention to detail and relationship-building ability. Experience in home health therapy and OASIS documentation.
