Home Health PT - Full Time

We are seeking a Physical Therapist (PT) to provide skilled home health therapy services. Key Responsibilities Include Conduct initial and ongoing assessments to evaluate mobility, strength, and functional capacity.Develop and implement personalized treatment plans aligned with physician orders.Deliver physical therapy interventions, monitor progress, and modify plans as needed.Document patient performance, treatment outcomes, and discharge plans accurately and timely.Educate patients and caregivers on home exercise programs and discharge procedures.Collaborate with interdisciplinary team members to optimize patient care.Supervise relevant personnel such as PT Assistants and Home Health Aides where applicable.Complete required assessments, including OASIS, in accordance with regulations. Qualifications Include Degree in Physical Therapy from an accredited program.Current, unrestricted state licensure as a Physical Therapist.Proficiency in clinical assessments, documentation, and compliance standards.Valid CPR certification and driver's license. Preferred qualifications: Experience in home health therapy and OASIS documentation.
