Home Health PT - Full Time

About the Role

Seeking a Physical Therapist (PT) to provide home health care services, enabling patients to recover in their own homes.

Responsibilities

• Perform assessments and develop personalized plans of care
• Update and deliver therapy per physician orders
• Document progress to ensure compliance and optimal outcomes
• Plan for patient discharge
• Educate patients and caregivers
• Coordinate with interdisciplinary teams
• Supervise clinical staff such as PTAs and HHAs where permitted
• Complete OASIS assessments in accordance with regulations
• Maintain effective communication with team members and families
• Adhere to all policies, standards, and licensing requirements

Requirements

• Degree in Physical Therapy from an accredited program
• Current, unrestricted state license
• Valid CPR certification, driver’s license, and auto insurance

Preferred Qualifications

• Experience in home health therapy and OASIS documentation
