About the role

We are seeking a compassionate and skilled Home Health Physical Therapist Assistant (PTA) to provide personalized, one-on-one therapy services in a home setting. This position is ideal for a PTA who enjoys working independently while helping patients regain strength, mobility, balance, and overall functionality in their daily environment. You will work under the supervision of a Physical Therapist to carry out individualized treatment plans for patients recovering from illness, injury, surgery, or chronic conditions. This role requires travel to patient homes throughout the service area.

Responsibilities

  • Provide home-based physical therapy services under the direction of a supervising Physical Therapist
  • Assist patients with therapeutic exercises, stretching, balance training, gait training, and mobility activities
  • Monitor patient progress and report changes in condition to the supervising Physical Therapist
  • Reinforce and carry out individualized treatment plans established by the Physical Therapist
  • Educate patients and family members on exercises, mobility techniques, safety precautions, and home exercise programs
  • Assist patients in improving strength, endurance, coordination, and functional mobility
  • Document patient visits, progress, and treatment responses accurately and timely
  • Collaborate with interdisciplinary team members including RNs, LPNs, CNAs, OTs, and SLPs to support patient goals and continuity of care
  • Maintain compliance with home health regulations and company policies

Requirements

  • Graduate of an accredited Physical Therapist Assistant (PTA) program
  • Active PTA license in the state of Virginia
  • Minimum of 1 year of PTA experience preferred (home health experience preferred)
  • CPR certification and TB baseline screening required
  • Strong communication and interpersonal skills
  • Good clinical judgment and ability to work independently
  • Reliable transportation and willingness to travel throughout the service area
  • Compassionate, patient-focused, and team-oriented mindset

Schedule

Full-Time / PRN (Flexible Scheduling Available)
