Home Health Physical Therapist - Salaried or Per Visit

About the role

We are seeking a Physical Therapist (PT) to provide clinical assessments, develop treatment plans, and deliver therapy interventions based on physician directives within a home health setting. The role involves collaborating with an interdisciplinary team, updating patients' progress, and ensuring optimal outcomes.

Responsibilities

• Conduct initial and ongoing evaluations of mobility, strength, and function.
• Administer tests and interpret results to set functional goals.
• Develop and document plans of care, including goals, progress, and discharge plans.
• Deliver therapy services in accordance with physician orders.
• Coordinate with healthcare team members, patients, and caregivers.
• Supervise personnel such as PT Assistants and Home Health Aides where applicable.
• Complete required assessments like OASIS, adhering to state regulations.
• Maintain accurate documentation to ensure compliance and reimbursement.

Qualifications & Skills

• Degree in Physical Therapy from an accredited institution.
• Current, unrestricted state license.
• Valid CPR certification, driver's license, and insurance.
• Preferred experience in home health therapy and OASIS documentation.
