Responsibilities

• Provide physical therapy services to patients in their homes
• Develop and implement individualized treatment plans based on patient needs
• Document patient progress electronically
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Adhere to all regulatory and insurance requirements

Requirements

• Current license to practice physical therapy in the state(s) where you will be working
• At least one year of clinical experience in home health or related field
• Bachelor's degree in Physical Therapy from an accredited program
• Passing scores on the National Physical Therapy Examination (NPTE)

Qualifications

• Strong interpersonal skills and ability to build rapport with patients and families
• Excellent communication and problem-solving abilities
• Ability to work independently and manage time effectively
• Flexibility to work various shifts including evenings, weekends, and holidays

Skills

• Proficiency in electronic health record systems
• Knowledge of home health care regulations and standards
• Effective leadership and team collaboration skills
