Home Health Physical Therapist (PT)

About the role

Aveanna is seeking a Physical Therapist (PT) to join our team. We are dedicated to providing compassionate and high-quality care to patients in their homes.

Responsibilities

• Develop and implement individualized treatment plans based on patient needs
• Perform physical therapy treatments and exercises to improve mobility and function
• Evaluate patient progress and adjust treatment plans as necessary
• Communicate effectively with patients, families, and other healthcare providers
• Document patient care activities accurately and in compliance with company policies

Requirements

• Current state licensure as a Physical Therapist
• At least 1 year of clinical experience in home health or related field
• Bachelor's degree in Physical Therapy from an accredited program
• Passing scores on the National Physical Therapy Examination (NPTE)

Qualifications

• Strong interpersonal skills and ability to build rapport with patients and families
• Ability to work independently and as part of a multidisciplinary team
• Excellent communication and organizational skills

Skills

• Proficiency in using electronic health record systems
• Knowledge of home health care protocols and standards
• Ability to manage time effectively and prioritize tasks
