Home Health Occupational Therapist OT Full Time

About the Role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician directives within the home health setting.

Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and function
• Develop and implement treatment plans
• Document progress and modify goals as needed
• Collaborate with healthcare team members
• Educate patients and caregivers for discharge planning
• Supervise OT Assistants and Home Health Aides where permitted
• Complete OASIS assessments
• Maintain accurate documentation
• Ensure compliance with policies

Requirements

• Degree in Occupational Therapy from an accredited program
• Current, unrestricted state license as an OT
• Valid CPR and driver’s license

Skills

• Strong organization, communication, and attention to detail
• Time management and relationship-building abilities
• Experience in home health therapy and OASIS documentation (preferred)
