Home Health Occupational Therapist Full Time

Position Overview We are seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. They will collaborate with an interdisciplinary team, providing updates to optimize patient well-being. Key Responsibilities Perform initial and ongoing assessments of mobility, coordination, strength, and function.Administer, evaluate, and interpret tests to establish functional goals and outcomes.Provide written evaluations and plans of care, outlining occupational therapy needs, goals, progress, and expected results.Deliver occupational therapy services per physician orders and document accordingly.Maintain and modify the plan of care, including discharge planning and patient education on home exercises.Coordinate services with healthcare team members, including physicians, nurses, and caregivers.Supervise relevant personnel (e.g., OTA, HHA) as applicable.Complete assessments such as OASIS where permitted by regulations. Qualifications & Skills Degree in Occupational Therapy from an accredited program.Current, unrestricted state license as an Occupational Therapist.Proficiency in clinical assessments, documentation, and compliance.Valid CPR certification, driver's license, and automobile insurance.
